Youth Sponsorship Information

Youth and Teen Sponsorship helps relocated youth/teens to become familiar with their new home at Fort Carson and assists school aged children and teens when moving to a new Post.

Our School Age Services and Youth Center are always looking for new sponsors. To participate please complete the Youth Sponsorship Form and fax it to 719-524-0194.  Or, if you'd like, you can mail it to our Child, Youth & School Services CLEOS Office at Bldg 1518, Prussman, Fort Carson, CO 80913.

In order to ensure compliance with Health Insurance Portability and Accountability Act (HIPPA) of 1996 and Privacy Act (PA) of 1974, the Youth Sponsorship Form has been removed from this page.  However, this form can be emailed, or if preferred, it can be picked up at our Parent Central Services office during normal business hours.
