Registration Packets:
In order to insure compliance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996 and Privacy Act (PA) of 1974, the registration packet and several other forms have been removed from this page.
To have the Registration Packet emailed, please CLICK HERE, or if preferred, these forms may be picked up at our Parent Central Services office during normal office hours. Please take time to review the information and print out the forms ahead of time.
Per OPORD 13-078, all forms need to be hand carried, or mailed, to our Parent Central Services Office. Please note: mailing does NOT complete your Registration. Please note: The mailing address and the building location are DIFFERENT. The mailing address is:

Registration Information:
To complete the registration process, you must provide the following information:
- Sponsor/spouse name, rank, duty station, duty phone number, AKO e-mail address and cell phone numbers.
- Proof of income (All LES's, pay stubs, and income records for the family)
- Two emergency contacts other than the sponsor and spouse. (must be local)
- Current and UP-TO-DATE Immunization/shot records for children fifth grade and younger.
- Physicals/health assesments for children fifth grade and younger. (Must be filed within 30 days)
- Deployment Orders, if applicable
- Military ID/DoD Card to show proof of eligibility
Waiting List:
Families can put their children on a waiting list prior to birth or as soon as they know they will be coming to Fort Carson. If the sponsor and/or spouse is military, please include the military rank in the name box on the form.
Special Needs:
Parent Central uses the information received from the registration packets to determine whether a child needs to attend a Special Needs Resource Accommodation Process (SNAP) meeting, which is overseen by the Exception Familiy Member Program (EFMP) and the Army Public Health Nurse. If required, the parent will attend a meeting and share information with an interdisciplinary team to determine appropriate placement and/or acommodations in CYS programs. Your child's registration cannot be completed without the required Medical Action Plan or Diet Statement, please bring current action plans (dated, signed, and stamped by physician within last year) to your registration appointment to expedite the process.
